$75,000-$82,000 per year

The Hospice Social Worker provides essential psychosocial support, counseling, and resources to patients and caregivers navigating end-of-life care.

Responsibilities for Hospice Social Worker

• Provide medical social work services to hospice patients and families in accordance with established standards of care.
• Assess patient and family psychosocial needs and develop appropriate support plans.
• Provide emotional support, counseling, and resources related to end-of-life care, grief, and caregiver needs.
• Collaborate with the interdisciplinary team to address patient and family goals, concerns, and care needs.
• Educate patients and families regarding available community resources, support services, and coping strategies.
• Maintain accurate and timely documentation of social work services provided.
• Support patients and families through issues related to illness, loss, and the dying process.
• Participate in care planning, team meetings, and quality improvement activities as needed.
• Maintain compliance with hospice policies, regulations, and professional standards.
• Perform other duties as assigned.

Qualifications for Hospice Social Worker

• Master’s degree in Social Work (MSW) from a program accredited by the Council on Social Work Education required.
• Current Social Work license in the state of Rhode Island required.
• Bachelor’s degree in Social Work from an accredited program required.
• Previous healthcare experience required; hospice experience preferred.
• Understanding of hospice philosophy and end-of-life care practices.
• Experience supporting patients and families through death, dying, and grief preferred.
• Strong verbal and written communication skills with excellent organizational abilities.
• Current CPR certification required.
• Valid driver’s license with reliable, insured transportation that meets state and organizational requirements.
• Ability to work collaboratively with patients, families, caregivers, and interdisciplinary team members.
