As an Evening Shift LPN (Licensed Practical Nurse) on our team, you will play a vital role in providing high-quality care to our residents. Responsibilities

Provide skilled nursing care to residents, including administering medications, performing treatments, and documenting patient data

Develop and implement individualized care plans in collaboration with the interdisciplinary team

Monitor and maintain accurate records of resident care, including progress notes, treatment plans, and medication administration

Collaborate with other healthcare professionals to ensure seamless transitions of care and effective communication

Provide emotional support and education to residents and their families

Participate in continuous quality improvement initiatives to improve patient outcomes and satisfaction

Requirements

Current LPN license in the state of Wisconsin

Strong communication and interpersonal skills

Ability to work effectively in a fast-paced environment with multiple priorities
