Job Summary

Freedom At Home is seeking a compassionate and motivated Physical Therapist (PT) to join our dynamic team of healthcare professionals. As a Physical Therapist with Freedom At Home, you will have the opportunity to work with patients in their homes, providing exceptional care and making a real difference in their lives. Responsibilities

  • Evaluate patients in their homes and develop individualized treatment plans
  • Provide skilled physical therapy services
  • Educate patients and caregivers on exercises, safety, and mobility
  • Complete OASIS and clinical documentation in a timely manner
  • Collaborate with physicians, nurses, and other healthcare professionals
  • Monitor patient progress and adjust treatment plans as needed
  • Promote safe, high-quality patient care and excellent outcomes

Qualifications

  • Graduate of an accredited Physical Therapy program
  • Current Michigan Physical Therapist license
  • Current CPR certification
  • Minimum of 2 years of PT experience preferred
  • Home Health experience is a plus but not required
  • Valid driver's license, reliable transportation, and auto insurance
