Responsibilities
- Evaluate patients in their homes and develop individualized treatment plans.
- Provide skilled occupational therapy services to improve independence and quality of life.
- Assess home safety and recommend adaptive equipment or modifications.
- Educate patients and caregivers on therapeutic exercises, self-care techniques, and safe use of assistive devices.
- Complete OASIS assessments and clinical documentation accurately and on time.
- Collaborate with physicians, nurses, and other healthcare professionals to achieve the best patient outcomes.
- Monitor patient progress and adjust treatment plans as appropriate.
- Promote safe, compassionate, high-quality patient care.
Qualifications
- Graduate of an accredited Occupational Therapy program.
- Current Michigan Occupational Therapist license.
- Current CPR certification.
- Minimum of 2 years of Occupational Therapy experience preferred.
- Home Health experience is a plus but not required.
- Valid driver's license, reliable transportation, and auto insurance.
