Full-Time Compensation

• Up to $110,000 annually , based on experience and qualifications
• $5,000 Sign-On Bonus for full-time employees

Per-Visit Compensation

• Start of Care (SOC): $175 per visit
• Resumption of Care, Recertifications & OASIS Discharges: $100–$125 per visit
• Routine Physical Therapy Visits: $80–$90 per visit
• Meetings: $40 per meeting

Mileage Reimbursement

• $0.58 per mile , reimbursed after the first 20 miles of the workday

Compensation is based on experience, qualifications, employment status, and applicable visit type.

What You'll Do

As a Home Health Physical Therapist, you will

• Provide skilled physical therapy services to patients in their homes
• Conduct comprehensive evaluations and develop individualized treatment plans
• Perform Start of Care assessments, recertifications, resumptions of care, and discharges as assigned
• Deliver therapeutic interventions focused on mobility, balance, strength, fall prevention, safety, and functional independence
• Educate patients, caregivers, and families on treatment plans and home exercise programs
• Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals
• Complete accurate and timely clinical documentation using Homecare Homebase (HCHB)
• Promote patient independence, safety, and overall quality of life

Qualifications

• Current, active Pennsylvania Physical Therapist (PT) license

Home health experience preferred

• Experience with Homecare Homebase (HCHB) preferred
• Strong clinical assessment, treatment planning, and documentation skills
• Excellent communication and interpersonal skills
• Ability to work independently while maintaining strong communication with the care team
• Reliable transportation and willingness to travel throughout the assigned territory

This position primarily serves patients throughout Montgomery County, PA
