Job Summary

Licensed Practical Nurse, Home Healthcare - Florence. This home-based role supports complex care needs for adult and geriatric patients while providing focused assessments, care-plan support, patient education, and clinical interventions within the scope of practice. The position offers weekday daytime scheduling, opportunities for professional growth, and meaningful one-on-one patient connections.

Job Details

• Schedule: Five 8-hour day shifts per week, Monday through Friday, from 8:00 a.m. to 4:30 p.m. or 8:30 a.m. to 5:00 p.m., for a minimum of 40 hours per week. Weekends are not scheduled, and after-hours call is not required.
• Required Qualifications: Licensed Practical Nurse; at least 2 years of home healthcare experience; hospice experience; prior travel experience; Basic Life Support (BLS) certification; required experience with the Epic charting system; required influenza vaccination, with medical or religious exemptions accepted; demonstrated ability to perform central line blood draws and dressing changes, colostomy and ileostomy care, urinary catheter insertion, phlebotomy, routine visits, simple dressing changes, wound and wound-vac care, wound-drain management, pain assessment and management, and patient and family education.
• Preferred Qualifications: Previous charge experience and community hospital experience.
• Additional Information: The role provides care to adults and geriatric patients in their homes, with approximately 5–6 visits per day. Responsibilities include focused assessments, care-plan development and progression, wound and ostomy care, laboratory draws, catheter and drain management, edema management, leg wraps, medication-management education, and coordination with registered nurses and an interdisciplinary care team. Patient education may address heart failure, diabetes, chronic obstructive pulmonary disease, cardiopulmonary conditions, pain management, orthopedic and neurologic topics, and complication prevention. Physical therapy, interpretation services, and social services are available for interdisciplinary support.
