As a Physical Therapist at Colonial Manor, you will play a crucial role in helping our residents regain strength, mobility, and independence through personalized therapy plans. Our mission is to provide exceptional care and services to our residents, and we are seeking a dedicated Physical Therapist to join our team.
Responsibilities
- Evaluate and treat patients with physical disabilities or injuries, focusing on improving mobility, strength, and range of motion
- Develop and implement individualized therapy plans in collaboration with other healthcare professionals
- Conduct physical therapy sessions, including exercises, stretches, and other interventions
- Monitor patients' progress, adjust treatment plans as needed, and document results
- Communicate effectively with patients, families, and healthcare teams to ensure continuity of care
