We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Chestnut Hill Lodge Health and Rehabilitation Center. Licensed Practical Nurse (LPN) Responsibilities
- Provide skilled nursing care to residents, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans in collaboration with interdisciplinary team members
- Monitor and report changes in resident condition to the Nurse Manager/Charge Nurse
- Communicate effectively with residents, families, and other healthcare professionals
- Participate in quality improvement initiatives to enhance the delivery of care
- Maintain accurate and timely documentation of resident care and services

Licensed Practical Nurse (LPN) Requirements
