Position Summary

We are seeking a dependable, customer-focused Weekend Admissions Coordinator to join our team at a skilled nursing and subacute facility in Orange County. This position plays a critical role in ensuring smooth and timely admissions, providing exceptional customer service to residents, families, hospitals, and referral sources.

The ideal candidate is highly organized, professional, responsive, and committed to maintaining excellent relationships with referral partners while supporting census growth and quality resident care.

This position requires consistent weekend availability, including holidays as needed. Opportunities for professional growth and advancement are available for high-performing individuals.

Schedule

• Weekend position (Saturday and Sunday)
• Holiday coverage required as needed
• Additional weekday hours may be available based on facility needs
• On-call availability may be requested periodically

Essential Duties & Responsibilities

• Coordinate and facilitate all weekend admissions and transfers.
• Review referral information for completeness and appropriateness of admission.
• Communicate with hospitals, case managers, discharge planners, physicians, and referral sources.
• Verify admission documentation and ensure regulatory compliance.
• Complete admission paperwork and obtain required signatures.
• Welcome residents and families and provide a positive admission experience.
• Coordinate with nursing, therapy, dietary, social services, and other departments to ensure admission readiness.
• Assist with insurance verification and collection of required demographic and financial information.
• Maintain accurate admission logs, census records, and referral tracking.
• Respond promptly to referral inquiries and admission requests.
• Conduct facility tours for prospective residents and families when appropriate.
• Assist with marketing and relationship-building efforts with referral sources.
• Participate in weekend census management and bed availability tracking.
• Ensure all admissions are completed in accordance with facility policies, state regulations, and federal guidelines.
• Support the facility’s mission of providing exceptional resident-centered care.

Qualifications

• Experience in healthcare admissions, skilled nursing, case management, marketing, customer service, or related field preferred.
• Skilled nursing facility experience strongly preferred.
• Knowledge of Medicare, managed care, and insurance authorization processes preferred.
• Strong communication and interpersonal skills.
• Ability to multitask and work independently.
• Excellent organizational and time-management abilities.
• Proficiency with Microsoft Office and electronic medical records.
• Professional appearance and demeanor.
• Reliable transportation and dependable attendance required.
