About the job
Caring Angels Home Care is looking for a full-time compassionate Social Worker (LMSW) for our Rehabilitation Center in Egg Harbor New Jersey. As Social Worker (LMSW) , you will play an integral role in the daily functions of our Center.
LMSW Responsibilities
- Guide , asses and communicate psychosocial status to the RN Case Manager and interdisciplinary group.
- Connect and evaluate medical needs, home situations, financial resources, and community support.
- Implement personalized social work plans and collaborate with your Bereavement Coordinator.
- Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
- Provide empathetic counseling to patients and families.
Qualifications
- Required: Master of Social Work from an accredited school of social work.
- Required: Licensed as a Social Worker from the State Board of Social Work
Experience
- Preferred: Minimum one (1) year experience as a social worker in a rehabilitation setting.
- Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred.
