About the job
Caring Angels Home Care is looking for a Licensed Practical Nurse to conduct Telehealth Visits from 2:00pm to 9:00pm for our Brooklyn office. This position will transition to hybrid after quality performance during training in office. As a Licensed Practical Nurse you will play an integral role in the day to day activities of our center. This position offers competitive Benefit Package.
Job description
- The LPN responsibilities include conducting 2:00pm to 9:00pm Telehealth patient visits and monitoring their conditions, conducting assesments, etc.
- You should be able to address all the caregivers’ concerns and make appropriate adjustments to the patients’ in-home management plan.
- Monitor medication and insulin, and completing blood pressure, glucose, urine, etc.
- Listening to the concerns of family members and answering their questions.
- Educating caregivers and family on the aftercare or ongoing care of the patient.
- Providing suggestions for improved healthcare to physicians and family members of the patient.
- Monitoring patient recovery and compiling reports.
