Occupational Therapist, per Diem- Auburn, CA

Care Central Professionals Home Health is seeking a dedicated Occupational Therapist (OT) to join our compassionate and patient-focused team in AUBURN, CA . This role offers the opportunity to provide one-on-one care that makes a real difference in patients’ lives - all in the comfort of their own homes.

Responsibilities

• Conduct skilled OT evaluations and develop individualized care plans.
• Provide interventions to improve safety, independence, and quality of life.
• Educate patients, caregivers, and families on adaptive strategies, equipment use, and home safety.
• Collaborate with our interdisciplinary team to ensure holistic, patient-centered care.
• Maintain accurate, timely, and compliant documentation.

Qualifications

• Current California OT license in good standing.
• Graduate of an accredited Occupational Therapy program.
• At least 1 year of clinical experience (home health preferred).
• Excellent communication and organizational skills.
• Valid driver’s license and reliable transportation.
