What You’ll Do
- Provide individualized care based on the client’s Plan of Care
- Monitor and document changes in the client’s condition
- Communicate proactively with the Director of Nursing and family
- Ensure safe, high-quality in-home care
- Build a meaningful, long-term client relationship

Requirements
- Active, unrestricted RN or LPN license in Indiana
- CPR certification & negative TB test or chest X-ray
- Previous nursing experience preferred
- Valid driver’s license and auto insurance
- Ability to lift up to 50 lbs. and assist with mobility as needed
