At Briarwood Nursing and Rehab in Flint, Michigan, United States , we are seeking a skilled and compassionate Registered Nurse (RN) MDS Coordinator/NAC to join our team.
About the Role
We are looking for a dedicated and experienced RN to coordinate the care of our residents by gathering and analyzing data to develop and implement effective care plans. As an MDS Coordinator/NAC, you will be responsible for ensuring that our residents receive the highest level of care, while also maintaining compliance with regulatory requirements. In this part-time role, you will have the opportunity to work closely with our interdisciplinary team to make a meaningful difference in the lives of our residents.
Responsibilities
- Coordinate the care of residents by gathering and analyzing data to develop and implement effective care plans
- Ensure that residents receive the highest level of care by working closely with the interdisciplinary team
- Maintain accurate and detailed records of resident care and services
- Conduct regular assessments to identify areas for improvement and implement changes as needed
- Collaborate with other departments to ensure seamless communication and coordination of care
- Stay up-to-date with regulatory requirements and maintain compliance with all relevant standards
- Participate in educational and training programs to stay current with industry best practices
Requirements
- Current RN license in the state of Michigan
- Experience in long-term care or related field
- Strong assessment and problem-solving skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
