Job Summary

We are seeking a compassionate and skilled Registered Nurse (RN) to join our team at Bayshore Nursing & Rehab. As a Registered Nurse, you will be responsible for providing direct patient care, administering medications, and monitoring patient conditions. Responsibilities

Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living

Monitor patient conditions, including vital signs and lab results, and report any changes to the healthcare team

Develop and implement individualized patient care plans, in collaboration with the interdisciplinary team

Educate patients and families on disease management and treatment options

Maintain accurate and timely records of patient care and treatment

Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes

Participate in quality improvement initiatives to enhance patient care and outcomes

Requirements

Current RN licensure in the State of Wisconsin
