Day Shift, 5x8-Hour (08:00 - 17:00).

Job Description

The Occupational Therapist will provide skilled home health visits to patients with a variety of conditions, including med/surg, cardiac, orthopedic, and respiratory issues, among others. This role requires the completion of 6-8 visits per day and documentation through the BayCare Forms and Ace Timesheet system. Additionally, a laptop and mobile internet device (MiFi) will be provided for necessary communications and documentation.

Responsibilities

• Conduct home health visits and develop plans of care tailored to patient needs.
• Perform assessments and interventions for varied diagnoses including COPD, CHF, and post-surgical patients.
• Complete required documentation and maintain communication through provided technology.
• Adhere to compliance requirements including COVID-19 vaccination and flu shot protocols.
• Engage in self-scheduling based on home health office needs.

Required Experience

• A minimum of 1 year of home health experience.
• Knowledge and practical application of OASIS guidelines preferred.

Certifications/Licensure

• Current BLS certification required.
• Active Florida or Compact license required at the time of submission.
• COVID-19 vaccination or approved exemption required.
• Flu vaccine required or declination with mandatory mask wearing.
