Job Description

The Occupational Therapist will be responsible for providing therapy services to patients in a skilled nursing facility. This includes evaluating patient needs, developing treatment plans, and implementing therapeutic interventions to promote recovery and rehabilitation.

Responsibilities

• Assess patient conditions and needs.
• Develop and implement individualized therapeutic treatment plans.
• Monitor and document patient progress throughout the rehabilitation process.
• Collaborate with interdisciplinary team members to ensure comprehensive patient care.
• Educate patients and their families on rehabilitation processes and goals.

Required Experience

At least one year of Skilled Nursing Facility (SNF) experience is required.

Certifications/Licensure

The position requires an active Occupational Therapist license in California and knowledge of NetHealth (Rehab Optima).
