Located near Brooklyn, NY
A home care agency located near Brooklyn, NY is currently seeking an experienced Care Coordinator . This role is responsible for coordinating home care services, supporting clients and families, and communicating with caregivers to ensure continuity of care.
Responsibilities of the Care Coordinator
- Coordinate home care services and caregiver schedules to meet client needs.
- Communicate with clients, families, and caregivers regarding schedules, care needs, and service updates.
- Address scheduling changes and assist with securing coverage to maintain continuity of care.
- Maintain accurate client records and documentation.
- Work closely with the agency’s team to support daily operations and address client concerns.
Qualifications
- Previous experience as a Care Coordinator or in a similar role.
- Private duty home care experience preferred.
- Strong organizational, communication, and coordination skills.
- Reliable and professional, with experience managing responsibilities independently.
- Bilingual English/Spanish preferred.
