Home Health Occupational Therapist (OT) & Certified Occupational Therapy Assistant (COTA) – Per Diem / PRN
OT Pay: $80 per visit or $90,000–$110,000 annually (based on caseload)
COTA Pay: $60 per visit
Location: O‘ahu, HI (Reliable transportation required)
Schedule: Flexible / Per Diem (PRN) – Build Your Own Caseload

We are currently hiring licensed Occupational Therapists (OTs) and Certified Occupational Therapy Assistants (COTAs) who are passionate about improving quality of life through skilled, patient-centered home health therapy.

What You'll Do
- Provide skilled occupational therapy services in patients' homes
- Perform initial evaluations, reassessments, and fall risk assessments (OT)
- Develop and implement individualized treatment plans
- Improve independence with activities of daily living (ADLs), mobility, safety, and home management tasks
- Educate patients and caregivers on adaptive techniques and fall prevention strategies
- Collaborate with nurses, therapists, physicians, and caregivers
- Maintain accurate documentation in accordance with Medicare home health standards
What We're Looking For
- Current Hawaii license as an Occupational Therapist (OT) or Certified Occupational Therapy Assistant (COTA)
- Minimum one year of clinical experience (home health experience preferred)
- Strong communication, organizational, and documentation skills
- Ability to work independently in the field
- Valid driver's license and reliable transportation
- Passion for serving seniors, kupuna, and local families
