About the Role

We are seeking a compassionate, motivated, and resourceful Certified Social Worker (CSW) to join our Social Services Department. This is a fast-paced, dynamic environment where no two days are the same. The ideal candidate is an independent problem-solver, eager learner, and adaptable professional who can manage competing priorities while providing empathetic, resident-centered support.

Key Responsibilities

• Conduct psychosocial, cognitive, and mood assessments, including BIMS and PHQ-9, as assigned.
• Assist residents with paperwork, benefits, insurance forms, and personal requests.
• Research community resources, coordinate housing applications, and facilitate referrals.
• Coordinate discharge planning and transitions back into the community.
• Participate in interdisciplinary care planning and conferences.
• Collaborate with residents, families, healthcare providers, and community agencies to coordinate care and resolve concerns.
• Provide emotional support, advocacy, and assistance navigating complex psychosocial needs.
• Maintain accurate, timely, and confidential documentation.

Essential Skills

• Excellent professional, written, verbal, and resident-centered communication skills.
• Proficiency in Microsoft Outlook, Teams, Word, and calendar management.
• Strong organization, time management, and attention to detail.
• Independent problem-solving, critical thinking, and initiative.
• Understanding of HIPAA, confidentiality, and secure handling of medical records.
• Familiarity with healthcare systems, Medicare, Medicaid, and insurance processes.
• Ability to complete assessments and maintain electronic health records.
• Empathy, cultural sensitivity, adaptability, and professional judgment.

Qualifications

Preferred

• Bachelor's degree in Social Work (BSW) and active NJ Certified Social Worker (CSW) credential.
• Experience in healthcare, skilled nursing, case management, or human services.
• Familiarity with PointClickCare (PCC).

Other Candidates Encouraged to Apply

Applicants with a bachelor's degree in psychology, sociology, human services, or a related field who are currently completing their final year of a Master of Social Work (MSW) program are encouraged to apply.
