Peachtree Centre is a 151-bed skilled nursing facility with 40 short-term rehab beds.

• Offers comprehensive short- and long-term rehabilitation and outpatient services
• Features a state-of-the-art rehab gym with community reintegration to help residents achieve their highest level of function

Located just 40 minutes from Charlotte, NC, and 30 minutes from Spartanburg, SC. Gaffney is a welcoming small town known for its Southern charm, affordability, and strong sense of community - a great place to work and call home.

⚙️ Responsibilities

• Review resident information and evaluate its impact on assessment and treatment.
• Conduct and document thorough functional assessments using standardized procedures.
• Develop and implement individualized treatment plans with achievable goals.
• Comply with evaluation, treatment, and documentation guidelines; maintain timely, accurate records.
• Instruct and supervise therapy assistants as needed.
• Enter treatment and progress notes for each session.
• Revise treatment plans as needed throughout the course of care.
• Document treatment in clear, concise discharge summaries.
• Orient residents to services and treatment procedures appropriate to their needs.
• Collaborate with nursing staff, residents, and families to maximize functional abilities.
• Educate residents and caregivers on the continuation of home exercise and therapy.
• Assist with referrals for additional services as needed and maintain knowledge of community resources.
• Participate in rehabilitation team conferences and other facility meetings.
• Access continuing education opportunities to maintain clinical competency.
• Participate in quality improvement initiatives.
• Follow regulatory, professional, and facility standards.
• Perform other duties as assigned to provide quality care.
• Understand payer reimbursement and allocate therapy resources efficiently.
• Support Ahava’s mission and project a professional image.
• Maintain expected productivity levels.

✔️ Qualifications

• Bachelor’s or Master’s degree in Physical Therapy
• Current PT license in the state of practice
• BLS/CPR certification (current or obtained during orientation)
• Strong communication, teamwork, and organizational skills
