Responsibilities

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with daily living activities
• Develop and implement individualized care plans to meet the unique needs of each resident
• Collaborate with the interdisciplinary team to identify and address resident needs, concerns, and goals
• Conduct routine assessments and report findings to the nursing team
• Maintain accurate and thorough records of resident care and services
• Support and educate residents, families, and caregivers on healthcare practices and best practices
• Participate in quality improvement initiatives and staff meetings to continuously improve the quality of care and services
