Job Summary

As a Licensed Practical Nurse on our night shift team at Windsor Rehabilitation and Healthcare Center, you will play a vital role in providing top-notch care to our residents. With new management and exciting benefits, this is an incredible opportunity to join a growing team of successful, happy caregivers. As a LPN, you will provide supervision of staff, ensure the health, safety, and welfare of all residents under your care, and help us achieve our organization's goal of improving the lives of those we serve.

Job Responsibilities

  • Provide direct patient care, including administration of medications, wound care, and vital sign monitoring
  • Supervise and direct care staff, ensuring proper care is provided and regulatory standards are met
  • Develop and implement individualized care plans for residents
  • Communicate effectively with residents, families, and other healthcare professionals
  • Participate in quality improvement initiatives to monitor and improve patient outcomes

Qualifications

  • High school diploma or equivalent
  • Current State License as a Licensed Practical Nurse
  • LTC/SNF experience preferred
