As a Licensed Social Worker at VNA Health Group, you’ll play a vital role in supporting patients and families facing medical, emotional, and social challenges. You’ll ensure that patients have the resources, education, and emotional support they need to navigate their healthcare journey – helping them maintain dignity, independence, and quality of life. What You’ll Do

• Support Patients and Families: Conduct psychosocial assessments to identify patient needs and develop individualized care plans.
• Coordinate Care: Collaborate with nurses, therapists, physicians, and community resources to ensure seamless, holistic support.
• Empower Through Education: Provide counseling, crisis intervention, and education to patients and caregivers.
• Advocate for Access: Connect patients and families with community resources, financial aid, and emotional support services.
• Document with Purpose: Maintain accurate, timely documentation to ensure continuity and quality of care.

Qualifications

What We’re Looking for in You

We’re searching for a dedicated professional who combines clinical expertise with compassion, empathy, and integrity.

• Current Licensed Social Worker (LSW) in the State of New Jersey (LCSW preferred).
• Master’s degree in Social Work from an accredited school.
• Minimum of 1 year of social work experience (homecare or healthcare experience preferred).
• Excellent communication and organizational skills.
• Proficiency with EMR systems and comfort with technology.
• Valid driver’s license and reliable vehicle – this is a field-based role.

Physical Demands/Working Conditions

• Flexibility to work both indoors and in the community.
• Frequent use of phone, computer, and clinical tools to document and communicate.
• Active engagement with patients, caregivers, and community partners – your presence makes all the difference.
