We are seeking a motivated and compassionate Licensed Social Worker (LSW) to join our interdisciplinary team. As a key member of our healthcare staff, you will have the opportunity to make a meaningful impact on the lives of our patients and residents.
Responsibilities
- Provide social work services to patients and families, including assessment, treatment planning, and counseling
- Develop and implement individualized care plans to address the psychosocial needs of our patients
- Collaborate with the healthcare team to ensure comprehensive care and communication
- Facilitate group therapy sessions and support groups as needed
- Maintain accurate and timely records of patient interactions and care plans
- Participate in multidisciplinary team meetings and provide education to the healthcare team on social work issues
Utilize emotional intelligence and respect to provide empathetic and compassionate care
Requirements
- Licensed Social Worker (LSW) in the state of Massachusetts
- Master's degree in Social Work (MSW) or related field
Strong communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
Knowledge of community resources and benefits
