Responsibilities

• Provide direct patient care to residents, including assistance with activities of daily living (ADLs), medication administration, and vital sign monitoring
• Develop and maintain positive relationships with residents, families, and interdisciplinary team members
• Participate in resident assessment, care planning, and documentation
• Collaborate with other healthcare professionals to ensure seamless coordination of care
• Maintain accurate and timely records of patient care
• Perform restorative care activities, such as ambulation, range of motion exercises, and toileting assistance
• Adhere to Spring Creek Healthcare Center's policies and procedures, as well as applicable state and federal regulations

Requirements

• Current CNA certification in the state of New Jersey
• High school diploma or equivalent required

Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment
