About the job
Job Summary
Our agency is expanding, and we are seeking a dedicated Speech Language Pathologist to provide in-home therapy services to patients. The ideal candidate will have experience in home health and be comfortable working independently while coordinating care with an interdisciplinary team. Responsibilities
- Perform comprehensive evaluations and OASIS assessments
- Develop and implement individualized care plans
- Treat patients with speech, language, cognitive, and dysphagia disorders
- Accurately document visits in accordance with Medicare guidelines
- Educate patients and caregivers on exercises and compensatory strategies
- Communicate effectively with physicians and care team members
Qualifications
- Master’s degree in Speech Language Pathology
- Active Florida SLP license
- Home health experience preferred
- Knowledge of OASIS documentation is a plus
- Possesses and maintains current CPR certification
- Valid driver’s license and reliable transportation
