About the Role

As a Physical Therapist , you will be responsible for designing and implementing individualized treatment plans to help our residents achieve their maximum potential. You will work closely with our multidisciplinary team to provide comprehensive care, including physical therapy, occupational therapy, and speech therapy. Your expertise will guide our residents in regaining strength, mobility, and independence, enabling them to return to their daily lives with confidence and dignity.

Responsibilities

• Develop and implement individualized treatment plans for each resident, considering their unique needs and goals
• Conduct thorough initial and ongoing assessments to identify physical therapy needs and progress
• Provide one-on-one and group physical therapy sessions to residents, incorporating a variety of techniques and modalities
• Collaborate with other members of the healthcare team to ensure comprehensive care and optimal outcomes
• Maintain accurate and detailed records of resident progress and treatment plans
• Participate in facility-wide initiatives to improve care quality, patient satisfaction, and resident outcomes

Requirements

• DOCTORATE degree in Physical Therapy from an accredited program
• 2 years of experience as a Physical Therapist in a sub-acute or long-term care setting
• $12,000 sign on bonus
• Strong communication and interpersonal skills, with the ability to work effectively with diverse populations
• Ability to work in a fast-paced environment, prioritizing multiple tasks and responsibilities
