Full-Time Social Worker – Subacute Unit
Hopkins Center for Rehabilitation & Healthcare is seeking a compassionate and motivated Full-Time Social Worker to join our Subacute Care Team. We are a 5-Star facility dedicated to providing exceptional care while creating a supportive, team-oriented environment for both our residents and staff.

Responsibilities
- Complete psychosocial assessments and social service documentation in accordance with state and federal regulations.
- Participate in interdisciplinary care planning and resident care plan meetings.
- Provide counseling, support, and education to residents and families.
- Coordinate discharge planning and community services.
- Assist residents and families with adjustment, coping, and transition needs.
- Maintain accurate documentation within the Electronic Medical Record (EMR).
Qualifications
- Bachelor's Degree in Social Work or related human services field required; Master's Degree preferred.
- Minimum one year of experience in healthcare, rehabilitation, or long-term care preferred.
- Experience with discharge planning and interdisciplinary team collaboration.
- Strong communication, organizational, and interpersonal skills.
- Proficiency with Electronic Medical Records (EMR).
