Hero Health Practitioners, LLC is seeking a compassionate, Nurse Practitioner (NP) to support our Southwest Missouri communities, including Joplin, Carthage, Roaring River(Cassville). We are looking for dedicated Nurse Practitioner (NP) professionals who are passionate about providing quality resident care with our skilled nursing, long-term care, and assisted living communities throughout our Southwest region.

Nurse Practitioner (NP) play a vital role in improving patient outcomes and delivering exceptional clinical care.

What You'll Do

As the primary on-site Nurse Practitioner (NP) provider for your assigned facility, you will:

• Provide comprehensive primary care to residents in skilled nursing, long-term care, and assisted living settings.

Required Qualifications

• Master's degree or higher from an accredited Nurse Practitioner program
• National board certification (FNP, AGNP, AGPCNP, or GNP)
• Active Missouri APRN and RN licenses
• Eligible to practice under a Missouri Collaborative Practice Arrangement
• DEA and Missouri BNDD registration (or ability to obtain)
• Medicare (PECOS) and Medicaid enrollment eligibility
• Current BLS Certification
• Valid driver's license, reliable transportation, and current auto insurance
• Experience using Electronic Health Records (EHR)

Preferred Qualifications

• Two (2) or more years of Nurse Practitioner (NP) experience
• Active Nurse Practitioner (NP) State licensure in Missouri
• Experience in Family Practice, Adult, Skilled Nursing, Long-Term Care, Geriatrics, Home-Based Primary Care, or Acute Care
• Experience with PointClickCare or other Long-Term Care EHR systems
• Knowledge of CMS Skilled Nursing Facility regulations
• Familiarity with value-based care models, ACOs, I-SNPs, and geriatric prescribing principles

Compensation

• Competitive Total Compensation: $125,000-$150,000
• Performance Incentive Program through Hero's Provider Incentive Compensation Plan
