Registered Nurse RN Home Health (PRN)

Empath Home Health , a member of Empath Health is seeking a compassionate PRN Home Health RN (Registered Nurse) to work with our Home Health team to deliver individualized care to patients in their homes in north Sarasota county.

Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, along with personal care support that assists with daily needs as part of a seamless continuum of care.

Locations: Home health visits in north Sarasota county (including North Sarasota, Longboat Key, University Parkway, Downtown Sarasota, Fruitville , and surrounding areas)

Schedule: 10+ home health visits per week. Required weekend/holiday on-call rotations.

What you'll Do

• Perform comprehensive patient assessments, including OASIS assessments at required intervals
• Develop, initiate, and revise individualized plans of care as patient needs evolve
• Deliver skilled nursing services and treatments as outlined in the plan of care
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies
• Collaborate with the interdisciplinary team to ensure seamless, holistic care

What You'll Need

• Graduate of an approved nursing program with an active Florida RN license
• Prefers BSN or MSN degree
• Minimum of 1 year of direct patient care experience; prior home health RN experience preferred
• Strong assessment, critical thinking, and patient education skills
• Willingness to participate in weekend and holiday on-call rotations
