Elm Manor Nursing and Rehab Center is seeking a licensed and compassionate Speech-Language Pathologist (SLP) to join our dedicated in-house rehabilitation team. As an integral part of our skilled nursing facility, you will provide evaluation, treatment, and care planning to patients recovering from illness, surgery, or injury. We offer a collaborative, patient-centered environment where your expertise makes a direct and lasting impact on the lives of those we serve.
Part-time or consistent PRN available
Responsibilities
- Evaluate and diagnose speech, language, communication, and swallowing disorders
- Develop and implement individualized treatment plans
- Collaborate with physicians, nursing staff, and rehab team to ensure quality care
- Document patient progress and maintain compliance with all regulatory guidelines
- Educate patients and families on therapy goals and techniques for carryover
- Participate in interdisciplinary team meetings and discharge planning
Qualifications
- Master’s degree in Speech-Language Pathology from an accredited program
- Current New York SLP license (or eligibility)
- Certificate of Clinical Competence (CCC-SLP) preferred
- Prior experience in a subacute/skilled nursing setting is a plus
- Strong communication, clinical, and organizational skills
- Commitment to patient-centered care and outcomes
