We are seeking a compassionate and skilled Occupational Therapist to join our therapy team at Brookwood Gardens Rehabilitiation and Nursing Center, a Skilled Nursing Facility dedicated to providing exceptional care to our patients. As a valued member of our team, you will play a crucial role in helping our residents achieve their highest level of physical, emotional, and cognitive function.
Responsibilities
- Develop and implement individualized occupational therapy plans to meet the unique needs of our residents
- Conduct evaluations and assessments to identify areas of strength and weakness
- Design and implement tailored interventions to improve functional abilities, such as mobility, ADLs, and cognitive function
- Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal outcomes
- Maintain accurate and timely records of patient progress and therapy sessions
- Participate in interdisciplinary team meetings to discuss patient care and develop effective treatment plans

- Bachelor's degree in Occupational Therapy or related field
