Schedule/Shift

5 × 8 hour shifts, with a schedule of Monday through Friday from 07:00 AM to 03:00 PM.

Job Description

This position involves providing home health services in the coverage area of York County, with the possibility of extending to territories within a 60-mile radius of the facility. The RN will engage in patient assessments, care planning, and provide ongoing support for patients in their homes.

Responsibilities

• Conduct Start of Care assessments.
• Perform Resumption of Care visits.
• Complete Recertification and Discharge patient evaluations.
• Document patient care using HomeCare HomeBase.
• Participate in routine visits for ongoing patient assessment.

Required Experience

• Recent home health experience required; must demonstrate proficiency in home health practices and documentation.
• Minimum of 30 points documented across various patient care visits, including Start of Cares, Resumption of Cares, Recerts, and routine visits.

Certifications/Licensure

• Active RN license is required.
• Experience with HomeCare HomeBase is mandatory.
