Responsibilities

• Plan, implement, and evaluate individualized nutrition care plans for residents, considering their medical, dietary, and psychosocial needs
• Conduct initial and ongoing assessments of residents' nutritional status and develop personalized diet plans to meet their unique needs
• Provide nutrition education and counseling to residents, families, and caregivers to promote healthy eating habits and disease prevention
• Develop and maintain a comprehensive understanding of the facility's menu and nutrition program, making recommendations for improvement as needed
• Collaborate with the interdisciplinary team to develop and implement resident care plans that address nutrition-related issues
• Monitor and report on resident progress, identifying areas for improvement and implementing changes as necessary
• Stay current with the latest research, guidelines, and regulations related to nutrition and dietetics, applying this knowledge to improve resident outcomes

Requirements

• Must be a Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) with a degree from an accredited program in dietetics or a related field
• Current registration with the Commission on Dietetic Registration (CDR)
• Ability to work effectively in a fast-paced environment with a high level of attention to detail and organizational skills
• Excellent communication and interpersonal skills, with the ability to work with residents, families, and healthcare professionals from diverse backgrounds
