About the Role

As an LPN - Post-Acute Care Specialist, you will be responsible for providing personalized care to our residents, including:

• Assisting with daily living activities such as bathing, dressing, and grooming
• Administering medications and treatments as prescribed by physicians
• Monitoring and reporting changes in resident conditions to nursing staff and physicians
• Collaborating with the rehabilitation team to develop and implement individualized care plans
• Providing emotional support and companionship to residents and their families

Responsibilities

• Provide direct patient care to a diverse population, including those with complex medical needs
• Participate in quality assurance and quality improvement initiatives to ensure excellence in patient care
• Communicate effectively with physicians, therapists, and other healthcare professionals to ensure seamless care coordination
• Maintain accurate and detailed records of patient care, including medication administration and treatment plans
