Cardinal McCloskey Community Services is seeking a dedicated and compassionate Registered Nurse to provide oversight and coordination of 29i health care services for children enrolled in our Enhanced Family Foster Care Program . This critical role ensures that children receive timely, comprehensive, and high-quality health care services while maintaining compliance with agency, city, and state requirements.

Responsibilities

• Ensure compliance with all agency, ACS, OCFS, and New York State 29i health care requirements.
• Monitor and coordinate admission, comprehensive, and annual physical examinations, including vision and hearing screenings.
• Ensure completion and documentation of HIV screenings/risk assessments and STD risk assessments.
• Provide prompt triage and follow-up for illnesses, injuries, and other health concerns.
• Maintain accurate health records and ensure all required medical consents are obtained and documented.
• Monitor and maintain EVOLVE records, ensuring all information remains current and compliant.
• Coordinate routine and specialized medical care, including referrals and follow-up appointments.
• Supervise and support Nurse Care Managers and oversee the health care management process.
• Review medical reports and ensure appropriate follow-through on recommendations, referrals, and health education needs.
• Coordinate consultations with specialists and collaborate with the Agency Pediatrician regarding children's health needs.
• Maintain ongoing communication with social work staff regarding children's medical status and treatment plans.
• Assist with the orientation, training, and professional development of Health Services staff.
• Prepare medical information and documentation required for ACS reporting and Family Court proceedings.

Requirements

• Registered Nurse with current New York State Licensure.
• Previous supervisory experience preferred.
• Previous experience providing health care management in a Foster Care setting preferable
