About the job

Empower Lives as a Supported Living Program Case Manager

Clinical Supervision toward LCSW licensure available for eligible ASWs

Join the Toolworks, where compassion meets innovation in supporting individuals with developmental disabilities. We're seeking an experienced, person-centered professional to oversee Supported Living services for adults with disabilities. Toolworks' Supported Living Program serves, the San Francisco, San Mateo County, and East Bay Areas. If you're passionate about making a real difference and have the drive to lead, we want you on our team!

About the Role

As a Program Case Manager, you will be at the heart of our mission, ensuring that our clients lead fulfilling lives in the communities of their choice. The Program Manager supervises Community Living Instructors and Leads, coordinates individualized services for a caseload of six to eight clients, monitors service quality and regulatory compliance, and supports clients in achieving greater independence. Your role will be pivotal in providing support, quality assurance, and effective management to enhance the well-being of those we serve.

Candidates with an MSW and active California ASW registration are encouraged to apply. Toolworks may provide BBS-qualified clinical supervision toward LCSW licensure for eligible employees. Qualifying experience will depend on assigned duties, the supervision plan, and California BBS requirements; not all hours worked may qualify toward licensure.
