Sangre de Cristo Community Care is hiring a Full-Time Certified Occupational Therapy Assistant (COTA) to join our growing Home Health team serving Pueblo and Cañon City .

You'll provide one-on-one care in patients' homes, helping individuals improve their independence, function, safety, and quality of life. Our Home Health program serves a diverse patient population, including pediatric patients and individuals with developmental disabilities.

What You'll Do

As a Home Health COTA, you'll work under the direction of a licensed Occupational Therapist to help patients achieve their individualized treatment goals.

You'll

• Provide occupational therapy interventions according to the established plan of care
• Help patients improve their ability to safely and independently complete activities of daily living
• Support patients in developing or regaining functional skills
• Observe patient progress and identify changes in condition
• Communicate progress, concerns, and changes promptly to the supervising Occupational Therapist
• Educate and support patients, families, and caregivers
• Complete accurate and timely clinical documentation
• Collaborate with therapists, nurses, physicians, and other members of the interdisciplinary care team

Requirements

• Graduate of an accredited Occupational Therapy Assistant program
• Current NBCOT certification, as applicable
• Current Colorado COTA license or eligibility for Colorado licensure
• Valid, unrestricted driver's license and reliable transportation
• Strong communication, organization, and time-management skills
• Ability to work independently in a home-based care environment
• Previous COTA experience preferred
• Home Health experience is a plus, but candidates with transferable therapy experience are encouraged to apply
• Flexibility to assist with weekend coverage when needed
