Weekend RN, part-time

Shift: 10-hour shift, Saturdays and Sundays

Longleaf Hospice and Palliative Care is locally-owned with offices located in Atlanta, Covington and Roswell, offering end-of-life care to those with life limiting illnesses in the communities we serve. As a Weekend RN, you will

• Complete an initial assessment of patient and family to determine hospice care needs; provide a complete physical assessment and history of current and previous illness
• Use health assessment data to determine nursing diagnosis
• Triage calls and assist patient/primary caregiver with problem resolution; counsel the patient and family in meeting nursing and related needs
• Make visits for symptom management and support as needed
• Modify patient treatment plans as indicated by patients' responses and conditions
• Maintain current, accurate, detailed reports in accordance with policy
• Monitor, record and report symptoms and changes in patients' condition to the IDG
• Provide health care instructions to the patient as appropriate per assessment and plan
• Obtain and receive physician orders as required
• Pronouncement of patients
• Other duties as assigned

Requirements

• Current license to practice as a registered professional nurse in Georgia
• Current and continued CPR certification
• 2 years of recent hospice experience
• Good verbal and written communication and problem solving skills
• Responsible valid driver’s license
• Reliable, insured, automobile for making home visits required
