Hospice Social Worker (LMSW) (LCSW) - PRN

About the Role

Touchstone Health is seeking a PRN Hospice Social Worker in San Antonio who is called to walk alongside patients and families during some of life’s most meaningful and vulnerable moments. In this role, you’ll provide compassionate, high-quality psychosocial care that supports not just clinical outcomes - but dignity, understanding, and peace.

Key Responsibilities

• Provide social work services to hospice patients in accordance with the physician’s plan of care
• Support home health patients as needed, ensuring continuity of psychosocial care across settings
• Support patients and families in navigating emotional, social, and environmental challenges
• Connect patients to community resources, financial assistance, and support services
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Assist with discharge planning and care transitions to ensure continuity and safety
• Document assessments, interventions, and care coordination accurately and in a timely manner

Requirements

What We’re Looking For

• Master’s degree in Social Work (MSW) from a CSWE-accredited program
• Active LMSW or LCSW licensure as required by the state of Texas
• At least one year of social work experience in a healthcare setting (hospice experience preferred)
• Strong skills in communication, assessment, and patient advocacy
• Deep empathy and the ability to navigate complex family dynamics with sensitivity and professionalism
• Detail-oriented, self-directed, and comfortable working independently in a field-based role
