About the job

The licensed practical nurse will support our psychiatric prescribers and help patients access and follow their prescribed treatment. This position will support PAX Health prescribers, serving as a consistent point of contact for medication questions, pharmacy concerns, prior authorizations, and nursing follow-up.

The nurse will work closely with prescribers, patients, pharmacies, and insurance companies to resolve medication barriers, coordinate ordered monitoring, and organize clinical information for prescriber review. This role requires strong communication skills, sound nursing judgment, and a compassionate approach to patient care.

Responsibilities

  • Respond to patient medication questions and concerns within the nurse’s scope of practice and established clinical protocols.
  • Gather and document information about reported side effects, medication adherence, and treatment concerns, escalating issues to the appropriate prescriber.
  • Coordinate medication prior authorizations, including submitting supporting documentation, tracking requests, and communicating outcomes.
  • Work with pharmacies and insurance companies to address coverage issues, prescription clarification requests, and other medication access barriers.
  • Coordinate refill requests for prescriber review and approval.
  • Track laboratory testing, vital signs, and other monitoring ordered by prescribers; follow up on outstanding items and route results for clinical review.
  • Reinforce prescriber instructions and provide patient education about medications, follow-up appointments, and ordered monitoring.
  • Conduct clinically indicated nursing follow-up and nursing visits as ordered and permitted within the nurse’s licensure and practice setting.
  • Recognize and promptly escalate urgent symptoms, safety concerns, or changes in patient condition according to practice protocols.
  • Maintain accurate, timely documentation in the electronic health record.
  • Communicate outstanding concerns and follow-up needs to prescribers and relevant team members.
  • Help develop consistent workflows for medication questions, prior authorizations, monitoring, and nursing follow-up.
  • Support documentation for eligible nursing visits in coordination with clinical and billing teams, including CPT 99211 when applicable requirements are met.
  • Other duties as assigned within the scope of the role.
  • Abide by PAX Health’s code of ethics, policies, and organizational values.
