How you’ll contribute

A Pharmacistwho excels in this role coordinates and provides pharmaceutical care for patients, reviews and verifies orders, manages drug therapy protocols, educates staff/patients, and supports quality and regulatory standards.. Additional responsibilities include :

• Manages drug therapy protocols as part of an interdisciplinary team.
• Reviews, interprets, and evaluates medication orders to identify, prevent, and resolve drug-related problems.
• Provides drug information and education to medical staff, healthcare professionals, patients, and pharmacy students.
• Documents interventions and outcomes for drug therapy quality improvement.
• Verifies medication orders for accuracy/completeness prior to entry; checks filled medications prepared by technicians; maintains proficiency with pharmacy automated systems and software.
• Manages patients in an outpatient anticoagulation clinic, including point-of-care testing, result evaluation, dosing, and scheduling.

What we’re looking for

Applicants should have a Doctorate degree in pharmacy and be a licensed Pharmacist in Utah. Additional requirements include :

• Basic Life Support certification is required; ACLS is preferred.
