We're looking for a skilled and caring Physical Therapist Assistant to join our team and help patients achieve their rehabilitation goals. This position is full time with hourly pay in the Peterborough/Keene area.

Responsibilities

• Implement patient care plans developed and supervised by a Registered Physical Therapist, delivering hands-on therapeutic services in patients' homes and community settings.
• Provide skilled therapeutic interventions to maintain, improve, or restore patient function across neuromusculoskeletal, pulmonary, and cardiovascular conditions.
• Collaborate with the care team including supervising therapists, nurses, physicians, and other providers to ensure coordinated, patient-centered care.
• Educate patients and families on therapeutic procedures, home exercise programs, and strategies to support recovery and independence.
• Document clinical progress accurately and in accordance with HCS standards and regulatory requirements.
• Monitor and ensure patient safety during all therapy sessions, relaying significant changes in patient status to the supervising PT in a timely manner.
