• Schedule: Days shift, 8 hours per day, with a minimum of 40 hours per week.
• Provides discharge planning services for adult Medical/Surgical patients.
• Generally assigned to one unit but may need to float to other units.
• Must be separated from BILH for 6 or more months to be considered and be in good standing.
• Epic experience is required.
• Dress code is casual or scrubs.
• Average daily caseload is 22-28.
• Five Case Managers are scheduled each day.
