Job Details

• Certifications, Education, and Experience Requirements: Must have a BLS certification through the American Heart Association, minimum of 2 years recent RN Case Management experience, including experience in a long-term acute care or specialty hospital setting, with strong care coordination and discharge planning skills across acute care environments. Must possess a California RN license.
• Schedule: Monday through Friday, 8:00 am to 4:30 pm, with a minimum of 40 hours per week.
• Additional Clinical/Operational Details: The position requires use of a specific template for application submissions, including detailed candidate information, experience, and certifications.
