JOB SUMMARY : A member of patient care team within the Hospice department and functions directly under the supervision of the Patient Care Coordinator and is directly accountable to the Patient Clinical Manager. The position requires that the individual assist the professional nursing staff by performing specific nurse aide functions related to patient care to include bathing, shampooing, ambulation, transfers, tidy sick area and other duties as identified on care plan under the supervision of a registered nurse.

Responsible for the secretarial aspects of patient care and the environmental management of the work unit including monitoring equipment and supplies, answering the phone, relaying messages, maintaining the cleanliness of the nurses’ station and other duties as delegated. Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served; also demonstrates knowledge of the principles of growth and development and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his/her age specific needs, and to provide the care needed. Demonstrates good customer service skills when interacting with patients, visitors, and co-workers. Prepares documentation accurately and timely according to policies and procedures. Assists with team meetings. CNA participates in in-services and competency programs as applicable. Performs other job duties as assigned.

PERFORMANCE EXPECTATIONS: Includes the following: Other duties may be assigned.

• Demonstrates ability to bathe patient in shower or bed using appropriate water temperature, preventing falls, and maintain privacy.
• Demonstrates ability to transfer patients safely utilizing safe body mechanics for your- self and patient.
• Demonstrates ability to document in the EMR according to policies and procedures.
• Demonstrates knowledge of changes in patient that require notification of nurse.

QUALIFICATIONS

Education/Skills: High School Diploma/GED and completion of an approved Nurse Aide training course required.

Work Experience : Six months of Nurse Aide work experience preferred. Computer experience required .

effectively and positively, maintain rapport in all situations, and adapt to varied

socioeconomic levels and environments.
