- POSITION SUMMARY
- The Senior Services Care Coordinator functions on behalf of the skilled nursing facility and the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being of each resident.
- POSITION QUALIFICATIONS
- Minimum Education
- Licensed Practical Nurse or Bachelor of Social Work
- Minimum Experience
- 2 years in nursing or medical social work
- Required Registration/License/Certification
- Active Alabama license by State Board of Social Work or Nursing
- Preferred Education
- BSN or MSW
- Preferred Experience
- 5 years nursing or social work experience.
- Preferred Registration/License/Certification
- N/A
