About the Role

At Corus Health, we are known for warm, steady, and mission-anchored care. As a Medical Social Worker on our hospice team, you'll walk alongside patients and families during some of life's most significant moments. • Connect patients and families with community resources, financial support, and social services
  • Help develop and carry out individualized care plans
  • Contribute to planning that supports safe, smooth transitions
  • Partner with community organizations and make referrals when they will help
  • Join patient care conferences and consult with your teammates
  • Document your assessments and interventions clearly and on time, in line with Medicare CoPs, state regulations, and professional social work standards
